This field comes in two different versions, Choice List and Choice List Multi Select. The first one is recommended for information where you need to select a single element from a list of options, while the second one will allow you to select multiple elements from that list of options.
In order to create a Choice List field, you must first create the Choice List with the options. To learn more about how to create it, please visit the following tutorial:

Navigating to Form Fields
Go to Settings > Apps, select the app you'd like to add the field to and select the 'Form Fields' tab.
Configuring a Choice List Field
The field type must be set to 'Choice List'.
1. Field Name
Give your field a name and add a field hint if desired. You can then select the section in which the field will be created or keep the default option.
2. Type
You can create a regular 'Choice list' or a 'Multi-Select' choice list. A multi-select choice list will allow your users to make more than one selection from your choice list.
3. Choice List
In the Choice List field, you will need to select the Choice List you created previously. See the following article to learn how to create and manage your Choice Lists.
Note: The same Choice List can be used to create different Choice List Fields.
4. Render As
You can choose to render your choice list as a dropdown field or as buttons.

5. Parent Choice List
Parent Choice Lists are useful when you might need to create a subcategory beneath the main one. For example, enabling the selection of subindustry based on industry selection within the same form.
6. Default Value
If needed you can choose a default value from your choice list to populate the field.
7. Required
Enable the 'Required' checkbox if the information for this field is mandatory. It means that if you enable this option, every time a team member creates a new record, they will need to input the information for this field in order to save it.
8. Lock After Create
If you enable the 'Lock After Create' as its name implies, the field will be locked once the new record is saved and the value of the field will not be editable afterward.
Once you are done setting your field, click the save button.
Note: All new fields are added at the bottom of the form.
Dependent Choice List
If you need to create a Choice List that shows different choice values based on a parent choice list selection, e.g. enabling the selection of a sub-industry based on industry selection within the same form, then follow the link below to learn how to.
In the Choice List field, you will need to select the Child Choice List you created previously. Once you do a new field will appear at the top of the form called 'Parent Choice List', click the dropdown to display the available options, and click on the Parent Choice List.
